Maintenance failures ruled not to be the cause of death: R -v- Otis Lifts

Otis was ordered to pay a total of £545,000 fines and costs for failure to maintain lifts in three local authority tower blocks after two men fell to their death through lift landing doors. 

The failings of Otis were not causative of the deaths.
